How to Reduce Appointment No-Shows: A Reminder Playbook

Josh Pocock

7 min read

No-shows cost service businesses twice: the slot sits empty, and the lead you paid for goes cold. Med spas, clinics, home service companies and sales teams all fight the same problem with the same tools: confirmations, reminders and fast rebooking.

You reduce appointment no-shows by confirming every booking right away, sending reminders that ask for a reply, making rescheduling easy, rebooking missed appointments within minutes and tracking show rate by lead source so you fix the right problem.

Below is the full cadence, the scripts we start from and how to measure whether it is working.

The two gold messages are the ones that ask the person to answer. Both have somewhere to send that answer, which is what turns a cancellation or a miss back into a booking.

What the research says about reminders

Useful published evidence on reminders comes from healthcare. A 2013 Cochrane systematic review, "Mobile phone messaging reminders for attendance at healthcare appointments" (Gurol-Urganci and colleagues), included eight randomized trials with 6,615 participants.

Across the included studies, attendance was 67.8% with no reminder, 78.6% with text message reminders and 80.3% with phone call reminders. The authors rated the evidence low to moderate quality and noted the studies were quite different from each other.

Keep the limits in mind. The trials are healthcare appointments, the review is more than a decade old and it did not test reminder timing or wording. It supports one conclusion: reminders help attendance, and texts performed about as well as calls in those trials. The cadence below is our practical playbook, not a finding from that review.

Step 1: confirm the appointment at booking

The confirmation goes out the moment the appointment is booked. It restates the details so there is no confusion later, and it sets expectations.

Hi [Name], you're booked with [Business] on [Day, Date] at [Time] at [Address or "by phone"]. [Provider or tech] will [what happens at the appointment]. Need to change it? Reply here or use this link: [reschedule link].

Include anything the person needs to do before the visit: arrive 10 minutes early, have the dog put away, bring photos of the damage, fill out an intake form.

If you book by phone, send the text before you hang up and ask the person to check it arrived. A wrong number caught now saves a no-show later.

Step 2: run a reminder cadence

The day before: ask them to confirm

This is the reminder that does the most work. It asks for a reply, which turns a passive message into a commitment and gives you a day's notice if they can't make it.

Hi [Name], reminder that you're scheduled with [Business] tomorrow at [Time]. Reply YES to confirm, or reply R to pick a new time.

Send it during working hours, not at 7 a.m. or 10 p.m.

The morning of: a short nudge

Morning [Name], see you today at [Time] at [Address]. Reply here if anything changed.

Keep it to one or two lines. People read these at a glance.

One hour before, or a travel-time text

For in-office appointments, a short text an hour before works well:

Hi [Name], see you in an hour at [Address]. Parking is [detail].

For in-home visits (roofing, HVAC, cleaning), send an on-the-way text instead. It tells the homeowner to be there and reduces the "nobody answered the door" no-show.

Hi [Name], [Tech] is on the way and should arrive around [Time]. Reply here if you need a few more minutes.

Handle the replies

Every reply needs an answer within minutes. A YES gets a short thank-you. An R or "can't make it" goes straight into the rescheduling flow below. A question gets answered or handed to your team.

If nobody watches these replies, the cadence fails quietly. This is where an AI agent helps most: our done-for-you AI appointment setters read every reply, answer basic questions, move appointments and hand off to a person when asked.

Step 3: make rescheduling easy

People who feel stuck with a time they can't make often just don't show. Give them an easy way out that keeps them on your calendar.

No problem, [Name]. I have [Day] at [Time] or [Day] at [Time]. Which works better?

Offer two specific times rather than a vague "when works for you?" Release the old slot immediately so you can fill it from a waitlist or a new lead.

Put the reschedule link in every message, and make sure it works on a phone without logging in.

Step 4: rebook no-shows within minutes

When someone misses an appointment, send a text 10 to 15 minutes after the start time. Waiting until the next day lets the moment pass.

Hi [Name], looks like we missed you at [Time]. No problem, things come up. Want to grab another time? [Day] at [Time] or [Day] at [Time]?

If there is no reply, try once more the next day, then stop. Do not lecture or charge in the text. A friendly rebook gets far more replies than a policy reminder.

Step 5: use deposits where they fit

Deposits or a card on file reduce no-shows for appointments that hold real capacity, like a 90-minute treatment slot at a med spa or a paid consultation. They are a poor fit for free estimates, such as a roofing inspection, where the ask would scare off good leads.

If you take deposits:

  • State the policy at booking, in plain words, before the person commits.
  • Repeat it in the confirmation text.
  • Say exactly when the deposit is refundable (such as rescheduling with 24 hours' notice).
  • Apply it to the visit so it does not feel like a fee.

Check any card-on-file or cancellation fee policy against your payment processor's terms and local rules. For a clinic or med spa setup, see how we work with med spas and clinics.

Step 6: measure show rate by lead source

A single show rate for the whole business hides the problem. Split it by where the lead came from.

Show rate = appointments attended ÷ appointments booked

Lead sourceBookedAttendedShow rate
Website form[ ][ ][ ]
Meta lead form[ ][ ][ ]
Phone call[ ][ ][ ]
Referral[ ][ ][ ]

Fill this in from your CRM for the last 90 days. If one source has a much lower show rate, the issue is usually upstream: weak qualification, a lead form that is too easy to submit or slow follow-up after the lead came in. Fixing the reminder cadence will not fix a source that books people who never meant to show.

Track the cost side too. Our post on cost per booked appointment covers the math for turning show rate into cost per attended visit.

Send reminders only to people who booked with you and gave their number for that purpose, and keep the wording about the appointment. Honor STOP and other opt-out replies right away, and keep your consent language in line with your SMS terms. This is general information, not legal advice, so confirm specifics with your own counsel.

FAQ

How do you reduce no-shows for appointments?

Confirm at booking, remind the day before with a request to reply, send a short reminder the day of, make rescheduling easy and rebook missed appointments within 15 minutes. Then track show rate by lead source.

When should you send appointment reminders?

A common cadence is a confirmation at booking, a reminder the day before that asks for a reply, a short text the morning of and a text about an hour before or when the tech is on the way.

Do text reminders reduce no-shows?

In a 2013 Cochrane review of healthcare trials, text reminders improved attendance compared with no reminder and performed about as well as phone calls. The evidence was rated low to moderate quality.

Should I charge a no-show fee?

Charge one only where the slot holds real capacity, like a long med spa treatment. Fees usually hurt bookings for free estimates. Spell out any policy at booking.
